An AI can find an old plan and report it as today's decision. I research how to help it keep track of what changed. The work is called epistemic memory: recording which decisions still hold, where sources disagree, and what nobody has answered yet.

Retrieval Is Not Enough: AI for Organizations Needs Epistemic Infrastructure
An AI can retrieve an old plan even after a new decision replaces it. OIDA records those changes in a graph: a set of connected records with labels for decisions, evidence, hypotheses, and open questions. It also stores disagreements and dates, so the system can use the organization's current position when answering.
